Few pointers for the form to make it easier to complete. Should take you less than 2 minutes to complete.
- For minors, please put the guardian’s name as well as their name in the participation box, it makes it much easier to fill out the form.
- The boxes may look like it all doesn’t fit, but they are unlimited, it will be “fine” on the database
- For adults you do still need to complete the section for minors, please list N/A or self, whichever you prefer.
- Please do not delete the email address, the address below the email address is for your physical address.

Please try to arrive 10 minutes prior to workshop starting time to complete check in and validate your waiver is on file. Also, you will be able to choose your materials when you arrive for the workshop. We offer a selection of colors however colors or unique materials may be limited and are available on a first come first serve basis. This helps us start class on time and provide adequate time to complete your projects! THANK YOU
